CLEAR offers 3 medical plans through UHC:
HDHP Core Plan - UHC Core Network
Surest Copay Plan - UHC Choice Plus Network
Choice Plus PPO Plan - UHC Choice Plus Network
If you enroll in a UHC medical plan, it automatically includes prescription drug coverage that is provided by Express Scripts (ESI).
After enrolling in the plan, you will receive two ID cards in the mail (UHC for Medical and ESI for pharmacy).

The HDHP Core Plan has the lowest premium per paycheck and covers preventive care at 100%.
For all non-preventive care, you'll pay the full cost billed by your provider until you meet your annual deductible. Once your deductible is met, you'll pay 20% of the cost billed by your provider, and this is called your coinsurance.

The Surest Copay Plan allows you to review costs in advance and compare care options directly in the Surest app. You literally "shop" for your provider!
Copay-only plan: this plan features transparent copays you can review before receiving care; no more confusing deductibles and coinsurance!
The higher quality care, the lower the copay!
Free telemedicine: $0 copays for telemedicine services.
Imporant note: This plan does not cover any out-of-network care except for emergency care.

The PPO Plan has the highest premium per paycheck and begins to share in the cost of medical services immediately through copays and co-insurances.
Both in- and out-of-network coverage is provided, although there is a preference for in-network so you'll pay less when you see an in-network provider.

Virtual visits can be conducted via the myUHC app or desktop site. Before you can conduct a virtual visit, you must first create a myUHC account. Instructions for creating an account can be found here.

Health plan price transparency helps consumers know the cost of a covered item or service before receiving care.
The Transparency in Coverage Final Rules require certain group health plans to disclose on a public website information regarding in-network provider rates and historical out-of-network allowed amounts and billed charges for covered items and services in two separate machine-readable files (MRFs).
