Dental Plan Options
You have a choice of two dental plans through Delta Dental. Both plans provide coverage for routine exams, x-rays, orthodontia services, cleanings, fillings, and major dental work (e.g., crowns, and bridges).
Dental PPO - Provides both in and out-of-network coverage.
Dental HMO - Provides in-network care only and requires the selection of a primary care dentist.
If you enroll in a Delta Dental plan, you will receive an ID card in the mail within 7 -10 business days.

If you elect the DHMO plan, you must select an in-network DeltaCare USA dentist, or else you may be charged 100% of the costs associated with your visit.
Providers are reimbursed based on a fee schedule with pre-determined dollar amounts, making it easy for you to predict costs.
If you wish to change dentists, you must do so online or by telephone by the 21st of the month for coverage to be effective by the next month. To find a DeltaCare USA dentist, you may call 1-800-422-4234 or by logging into the Delta Dental portal at deltadentalins.com
