Fertility and Family Planning coverage is a benefit offered to all full time team members and their eligible dependents. Level of coverage is dependent on whether the employee is enrolled in CLEAR's medical plan or not, along with employee tenure.

Eligibility: Team member must be enrolled in CLEAR's United Healthcare Medical plan. Team member's spouse/domestic partner is also eligible for this benefit if they are enrolled in our United Healthcare Medical Plan.
Coverage: Up to 3 KindCycles for fertility services and 1 year of storage and up to $15,000 for fertility medication via KindBodyRx. Limits are per lifetime.
Costs: Your plan's regular cost-sharing (i.e. deductible, coinsurance, and copays) will apply.
Eligibility: Team member must be enrolled in Kaiser plan or waived medical coverage. Coverage is for team members only and is not available to dependents.
Coverage: Up to 1 KindCycle for fertility services and 1 year of storage and up to $5,000 for fertility medication via KindBodyRx. Limits are per lifetime.
Costs: You will need to satisfy the $1,700 fertility deductible before benefits are payable.
Additional Coverage through Kaiser: Team members enrolled in our Kaiser medical plans may also have additional coverage through their medical plan. Please contact Kaiser directly at the phone number on the back of your ID card to learn more.
CLEAR provides the following benefits to team members who have been continuously employed for 1 year:
Up to $10,000 reimbursement per child for adoption, for donor and surrogacy services.
Please note, there is a $20,000 lifetime maximum per team member.
KindCycles include fertility treatment services such as: in vitro fertilization (IVF), intrauterine insemination (IUI), egg/embryo/sperm preservation, donor services and Kindman “male fertility” services; up to one (1) years of storage will be covered with applicable KindCycle. If you are enrolled in one of our UHC medical plans, your plan's normal deductible, coinsurance, and copay will apply, with the exception of any preventive care. Preventive care, such as an annual gynecological exam, is covered 100% by your health insurance.
If you are not enrolled in our UHC medical plan, you will be required to satisfy the $1,700 fertility deductible before benefits are payable.
Please note, there are some supplemental services offered by Kindbody that are self-paid and not covered by insurance. Please contact Kindbody directly to confirm if a service is covered by insurance and if covered, what your cost share would be.
