Kaiser medical plans offered at CLEAR automatically include prescription drug coverage. After enrolling, you will receive an ID card in the mail. All Kaiser medical plans use a Provider Network, offering no out-of-network coverage.

Our Bronze Plan is the most affordable plan option and covers preventive care at 100%.
For all non-preventive care, you'll pay the full cost billed by your provider until you meet your annual deductible. After that, you'll only pay 40% of the cost billed by your provider, which is your coinsurance.

If you work in the state of Hawaii, you are eligible to enroll in our Kaiser Medical Plan. The plan provides 100% coverage for in-network preventive services and access to expert medical care.

$15 Primary Care Physician Visits
Under this plan, you must use Kaiser doctors, pharmacies, and facilities, and your specialty care is subject to referrals from your primary care physician.
This plan automatically include prescription drug coverage. After enrolling in the plan, you will receive an ID card in the mail. All Kaiser medical plans use a Provider Network; this plan offers no out-of-network coverage.
