Benefits Eligibility
Team Member Eligibility
All full-time (working 30+ hours per week*) team members are eligible for CLEAR Benefits.
*Hours worked by part-timers will be reviewed annually in compliance with the Affordable Care Act. Based upon this review, anyone deemed eligible for healthcare benefits will be contacted directly.
Corporate and Salaried team members
All regular, full-time, salaried team members are eligible for all benefits upon hire. The benefits will become effective as follows:
Healthcare benefits will be effective on your date of hire
401k retirement benefits will be effective on the 1st of the month following your 90th day of employment
Field Hourly team members outside of California
Healthcare benefits will be effective on your 31st day of employment
401k retirement benefits will be effective on the 1st of the month following your 90th day of employment
Part-time team members outside of California are eligible for select benefits, including:
Dental and Vision - will be effective on your 31st day of employment
401(k) Retirement Plan - eligible 1st of the month following 90 days of employment
Hourly team members who work in California and Hawaii
All California and Hawaii team members, including part-time team members, are eligible for CLEAR Benefits.
Healthcare benefits will be effective on your 31st day of employment
401(k) retirement benefits will be effective on the 1st of the month following your 90th day of employment.
Individuals who may not be eligible to participate in CLEAR benefits include contractors, consultants, part-time team members working less than 10 hours per week, temporary/seasonal employees, agency employees, and interns. You will be informed during the hiring process if you are eligible for CLEAR Benefits.

Dependent Eligibility
Eligible dependents include:
Spouse: your legal spouse to whom you are lawfully married and not legally separated.
Domestic partner: an individual of the same sex or opposite sex with whom you’re in a domestic partnership, civil union, or other similar formal relationship that’s not a marriage under state law. More details are included below.
Children: up to age 26, plus disabled dependent children of any age who meet plan criteria.
Supporting documentation:
Supporting documentation will be required to enroll your eligible dependents and must be submitted within 30 days of your hire date or qualifying life event. Acceptable forms of documentation include:
Spouse: marriage certificate
Domestic partner: certificate of domestic partnership or completion of the Declaration of Domestic Partnership form
Child: birth certificate, decree of adoption, or proof of legal guardianship.

Domestic Partners
For a domestic partner to be eligible for CLEAR Benefits, you must satisfy one of the two requirements:
Your relationship has been recorded, certified and/or registered by a national, state, city or regional government authority.
You meet CLEAR's definition of eligible domestic partner as outlined in our Declaration of Domestic Partnership Form.
If your partner meets the eligibility requirements and you wish to enroll them in CLEAR benefits, please submit supporting documentation to The Benefits Help Desk at CLEARbenefits@onesourcevirtual.com.
