We're excited to offer a new, affordable plan option with copays for office visits and prescription drugs. Team members in Georgia (ATL Metro), Mid-Atlantic States, Oregon, and Washington can enroll in the new Kaiser plan or any of the United Healthcare plans.

United Healthcare Medical premiums will increase slightly
Individual coverage: $5 - $12 per month increase
Family coverage: $16 - $38 per month increase
High-Deductible Health Plan (HDHP) plan limits are increasing
Due to IRS mandates, there are higher deductibles and out-of-pocket maximums for the HDHP.
In-Network Deductible: $1,600/$3,200 -> $1,650/$3,300
In-Network Out-of-Pocket Maximum: $3,200/$6,400 -> $3,300/$6,600
Out-of-Network Deductible: $3,200/$6,400 -> $3,300/$6,600
Out-of-Network Out-of-Pocket Maximum: $6,400/$12,800 -> $6,600/$13,200
Dental & Vision premiums are not increasing
Enhancement to Dental PPO Plan
Preventive care will no longer count toward the $1,500 annual benefit maximum, giving you more coverage for major dental expenses.
For example, if each routine cleaning costs $150 and you have 2 cleanings per year, that’s an additional $300 in benefit coverage available for other dental needs. Visit costs will vary by region and dentist.

Bronze and Silver individual coverage premiums will not increase.
All other plan premiums will increase slightly by $8 - $38 per month.

Great news! Your healthcare premiums are staying the same, and we’re excited to introduce two new enhancements to your benefits package:
Triple S Medical Plan: $0 copays for office visits and diagnostic care received at Salus facilities
Delta Dental PPO Plan:
Preventive care will no longer count toward the $1,500 annual benefit maximum, giving you more coverage for major dental expenses.

$0 plan premiums will remain free.
Anyone currently paying for family coverage will see an increase of $20 - $37 per month.
New for 2025:
Chiropractic coverage for up to 20 visits annually at $20 per visit.
Enhanced $200 Fit Rewards Gym Membership or $0 Home Fitness Program.

Increased IRS Contribution Limits for Tax-Favored Accounts
The maximum amount you can contribute to tax-favored accounts in 2025 is:
Health Savings Account:
Individual: $4,300 ($150 increase from 2024)
Family: $8,550 ($250 increase from 2024)
Healthcare & Limited Purpose Flexible Spending Account:
$3,300 ($100 increase from 2024)

Field Salaried: Long-Term Disability Enhancement
CLEAR covers the cost of Long-Term Disability Insurance, which provides financial support by replacing a portion of your income if you’re unable to work for an extended period due to illness or injury.
Good news! We're increasing the maximum monthly benefit from $1,000 to $5,000 for Field Salaried team members.
This change will go into effect for any disabilities that occur after January 1, 2025.
