How do I request a copy of my insurance ID card?

You will need to contact the insurance carrier directly to request a replacement. Carrier contact information can be found here.

Alternatively, you can download the mobile apps for our insurance carriers to access a digital copy of your member ID card. Instructions for accessing your digital ID card can be found here.

How do I view my current benefit elections?

When do benefits end?

Termination of Employment

Upon termination of employment, your Medical, Dental and Vision coverage will continue through the end of the month in which you terminate. If you are terminated on the last day of the month, your coverage will end that day. You will be eligible to continue coverage under COBRA and will receive a COBRA packet in the mail from our COBRA administrator, ConnectYourCare. 


YOU Stipend

Benefits end upon separation. As of your resignation or termination date (whichever comes first), you will no longer be able to submit claims or make purchases using the Forma card or store.


CLEAR Membership

Separation Agreement for Hourly Team Members: 

After separation, your free Clear membership will be terminated effective on your last day of employment.  If you added any family members at a discounted rate, their accounts will be charged our standard family rate. Your family may apply alternative discounts, like Delta or United loyalty programs, or cancel their membership.

Separation Agreement for Salaried Team Members: 

After separation, you will continue to have access to your free CLEAR membership for 6 months after your separation date. After 6 months, your account will be converted to a standard rate membership and will be charged $189/year.  If you added any family members at a discounted rate, their accounts would also be charged our standard family rate. You may apply alternative discounts, like Delta or United loyalty programs, or cancel your membership. To avoid a charge, you may cancel your membership within the 6 month termination period. 

Status Change from Full-Time to Part-Time / Reduction in Hours

If you are formally changed from full-time status to part-time status, your Medical coverage will end effective the date of your status change. You will be eligible to continue coverage under COBRA and will receive a COBRA packet in the mail from our COBRA administrator, ConnectYourCare.

If you do not undergo a formal status change to part-time, but regularly work less than 30 hours per week, and to the extent allowed by law, CLEAR reserves the right to modify, revise, or terminate benefits at any point in time.

Note: This does not apply to individuals based in locations where part-time team members are eligible for medical benefits.

Unpaid Leave of Absence

If you are out on an unpaid leave of absence, FMLA, local, state, or other leave entitlement or program, or other CLEAR-approved leave, your benefits (with some exceptions) will continue during your leave of absence, up to 180 days. Upon returning to work, you will be payroll deducted for unpaid premiums that accrued while you were out on leave.

If you do not return to work after exhausting your approved leave of absence, your employment may be subject to termination. If your employment is terminated, your healthcare coverage will continue through the end of the month in which you terminate. If you are terminated on the last day of the month, your coverage will end that day. You will be eligible to continue coverage under COBRA and will receive a COBRA packet in the mail from our COBRA administrator, ConnectYourCare.

If you become disabled

CLEAR provides eligible team members short-term disability benefits for up to 180 days. Your healthcare and life insurance benefits will continue for the duration you are approved for short-term disability.

If you need to be out longer than 180 days, Unum will review your claim for long-term disability.

If you are out for 180 days or longer, your healthcare and life insurance coverage will end at the end of the month in which you reach 180 days of consecutive leave. You may be eligible to continue coverage under COBRA and will receive a COBRA packet in the mail from our COBRA administrator, Optum Financial (formerly ConnectYourCare). This packet will contain instructions for how to enroll in COBRA coverage and pay your premiums.

If you are approved for long-term disability, you may also qualify for waiver of premium for life insurance. If approved, your life insurance will continue while you are out on long-term disability. If your waiver of premium claim is denied by Unum, you will have the option to convert your life insurance coverage to an individual policy.

Continuation of coverage under COBRA

If your CLEAR benefits end, you may be eligible to continue coverage under COBRA  (Consolidated Omnibus Budget Reconciliation Act). You can learn more about COBRA here.
