After you enroll in CLEAR benefits or make changes to your benefits, you will sometimes receive an ID card in the mail, but not for all plans. Please review the information below for each insurance carrier to know when to expect your ID card, how to register online, access your digital ID card, download carrier mobile apps, and more!
If you have misplaced your insurance ID card you will need to contact the insurance carrier directly. The Benefits Team cannot replace lost ID cards. Please refer to the carrier directory for contact information.

Delta Dental
How to create an account
Go to deltadentalins.com
If you are on the computer, click on Log In (upper right corner) > Create an account.
If you are on your phone, click on the three lines in the upper left corner > Log In > Create an account
User Type: Enrollee/Adult Dependent
Fill in the information
Enrollee ID would be your SSN

Obtaining an ID card
DHMO participants: upon your initial enrollment in the DHMO plan, you will receive a letter with your ID card and assigned dentist. This is the only dentist you can see and use your dental benefits. If you wish to change your dentist, you must contact Delta Dental.
PPO participants: you will not receive an ID card in the mail
When at your dentist’s office, please let them know that you are enrolled in Delta Dental PPO or HMO with CLEAR and provide them with your name, date of birth, and Social Security number.

How to Create an account
Visit eyemed.com and click on Login --> Member
If you’re a new user, click on Need to Register?
New users will need to create an account using your member ID or the last four digits of your social security number
You’ll then receive a registration email in your inbox to confirm your account.
After registration, you’ll be able to set up your new account. Enter your email and desired password.
Confirm your password and hit Register for instant access
Once your profile is all set up, you can manage your profile at any time from the Manage Profile link at the top right of the page.
Obtaining your ID card
For vision, you will not receive an ID card in the mail.
Log into the Member Web, navigate to View Your Benefits and then Print ID Card

Go to kp.org/registernow from a computer (not a mobile device), click on Create my account and follow the sign-on instructions
You’ll need your medical record number/health record number, which you can find on your Kaiser Permanente ID card.
You will receive your ID card via mail shortly after enrolling in CLEAR’s medical plan.

Visit shieldbenefits.com, and the site will prompt you to enroll. You will need to create an account even if you have used LegalShield in the past as this is a new site.

Optum – Health Savings Account, Flexible Spending Account and Commuter
How to Create an Account
Access your account at www.connectyourcare.com.
If you elected a health care flexible spending account or health savings account:
You will need to wait until you receive your payment card in the mail to register.
Once received, access your account at www.connectyourcare.com.
Go to Sign Up -> I'm an employee -> Activate my account
Input your SSN, date of birth, and the number of your payment card and submit. Choose a personal username and password on the next screen.
If you elected only a dependent care flexible spending account and/or commuter benefits
You can access your new account today at www.connectyourcare.com.
Go to Sign Up -> I'm an employee -> Activate my account. Input your SSN and DOB, check the box for “My account does not have a payment card associated with it” and submit.
How to Enroll In Commuter Benefits
See above instructions to create and access your account at www.connectyourcare.com.
Once your account is created, go to Commuter Benefits on the left-hand side
Ensure your pop-up blocker is disabled if the commuter portal does not open
Choose Place an Order in the left-hand menu and follow the onscreen instructions to complete your order
Orders are due the 10th of each month prior to the benefit month (i.e. orders for the month of March must be placed by February 10)
