Fertility Treatment & Preservation Coverage
United Healthcare Enrolled Team Members
Up to 3 KindCycles for fertility services and up to $15k for fertility medication via Schraft’s pharmacy. Limits are per lifetime.
Your spouse/domestic partner is also eligible for this benefit if they are enrolled in our United Healthcare Medical Plan.
Your plan's regular cost-sharing (i.e. deductible, coinsurance, and copays will apply).
All other team members:
Up to 1 KindCycle for fertility services and up to $5k for fertility medication via Schraft’s pharmacy. Limits are per lifetime.
Coverage is for team members only and is not available to dependents.
You will need to satisfy the $1,500 fertility deductible before benefits are payable.
Team members enrolled in our Kaiser medical plans may also have additional coverage through their medical plan. Please contact Kaiser directly at the phone number on the back of your ID card to learn more.
You are eligible for this benefit after 1 continuous year of employment.
Adoption & Surrogacy Assistance
CLEAR provides the following benefits to team members who have been continuously employed for 1 year:
Up to $10,000 reimbursement per child for adoption, for donor and surrogacy services.
Please note, there is a $20,000 lifetime maximum per team member.

What is Kindbody?
Kindbody offers fertility, gynecology, and wellness services in modern, tech-enabled clinics. As a Kindbody member, you can expect best-in-class care, accessible pricing, and a seamless patient experience.
Who is eligible for Kindbody Services?
Available to all CLEAR Team Members:
Voluntary egg freezing
Available to all CLEAR Team Members and their Spouse's After 1+ year of employment:
$10,000 reimbursement per child/per year for adoption, for donor and surrogacy services
Terms and Conditions:
One (1) year from date of hire, when covered under a CLEAR sponsored medical plan or if you have waived medical coverage.
$20k Lifetime Maximum per team member.
$15k Medical Prescription Lifetime Maximum per team member.
Team Members enrolled in CLEAR UHC Medical Plan(s) ONLY:
Fertility treatment services (i.e. IVF, IUI, etc.)

What is my cost share for Kindbody fertility services and medications?
If you are enrolled in one of our UHC medical plans, your plan's normal deductible, coinsurance, and copay will apply, with the exception of any preventive care. Preventive care, such as an annual gynecological exam, is covered 100% by your health insurance.
If you are not enrolled in our UHC medical plan, you will be required to satisfy the $1,500 fertility deductible before benefits are payable.
Please note, there are some supplemental services offered by Kindbody that are self-paid and not covered by insurance. Please contact Kindbody directly to confirm if a service is covered by insurance and if covered, what your cost share would be.
