Our Core Plan is the most affordable plan option and covers preventive care at 100%.
For all non-preventive care, you'll pay the full cost billed by your provider until you meet your annual deductible. After that, you'll only pay 20% of the cost billed by your provider, and this is called your coinsurance.

Surest Copay Plan
Surest Copay Plan - Exclusive Provider Organization (EPO)

The Surest Copay Plan allows you to review costs in advance and compare care options directly in the Surest app.
Copay-only plan: this plan features transparent copays you can review before receiving care; no more confusing deductibles and coinsurance! You can research providers and the cost directly within the Surest app.
Free telemedicine: $0 copays for telemedicine services.
This plan does not cover any out-of-network care except for emergency care.

This is our richest, but also most expensive, medical plan option.
Both in- and out-of-network coverage is provided, although you'll pay less when you see an in-network provider.
You'll pay copays for office visits and prescription drugs.
You'll pay all costs for major medical care until you meet your annual deductible, after which you'll pay 10% coinsurance (in-network).

All CLEAR medical plans include coverage for virtual visits so you can connect with a doctor via phone or video chat 24/7. All CLEAR medical plans include coverage for virtual visits or telemedicine. Virtual visits can be conducted via the myUHC app or desktop site. Before you can conduct a virtual visit, you must first create a myUHC account. Instructions for creating an account can be found here.

Travel Coverage
Medical Travel Benefits:
To help support our team members’ access to comprehensive health care, we have added a medical travel benefit to our United HealthCare (UHC) medical plan. Specifically the plan now offers travel reimbursement (transportation and lodging) for any plan member (team member, spouse or dependent) who cannot receive the medical care they need within 50 miles of their home. Care is comprehensive and includes, but is not limited to, family planning and reproductive health services. You can learn more about the details of this benefit by contacting UHC directly at 1-866-633-2446.

The objective in making this change is to ensure that team members, regardless of where they are located, can access the medical care they need.
UHC Travel Benefit:
Travel & Lodging is included for any Covered Health Care Service (including Abortion), that cannot be accessed within 50 miles of the participants home. Subject to plan criteria and Limited to $2500 per calendar year for these services.

Health plan price transparency helps consumers know the cost of a covered item or service before receiving care.
The Transparency in Coverage Final Rules require certain group health plans to disclose on a public website information regarding in-network provider rates and historical out-of-network allowed amounts and billed charges for covered items and services in two separate machine-readable files (MRFs).
